Chairman of the board presents with massive scrotal swelling after a candiru fish (penis fish) swam up his urethra while urinating in the Amazon River. The fish lodged in his prostatic urethra causing urinary obstruction for three days. Required surgical removal via open approach after endoscopic attempts failed, complicated by cardiac arrhythmias during surgery.

Pregnant amnesiac patient at 30 weeks gestation develops severely elevated blood pressure (171/118, later 202/134) with edema. Despite epidural placement to lower BP, she has a seizure and requires emergency C-section to prevent maternal death and deliver premature baby.
