Central case of the episode featuring a pregnant woman with a ruptured middle cerebral artery aneurysm causing strokes and aphasia. Derek performs an EC-IC bypass which initially appears successful, but the bypass fails leading to catastrophic bleeding. Despite multiple attempts to save her including temporal and frontal lobe resections, the patient dies after emergency C-section delivery of her baby.
