Meredith Grey remains critically ill and unconscious from COVID-19 with multiple complications. Despite stable vitals and improving chest X-rays, she develops hepatic vein and IVC thrombosis causing elevated liver enzymes and altered consciousness. A TIPS procedure is performed to remove the clot, but she still won't wake until her daughter Zola visits.

Felix is four months post-surgery for a rotator cuff tear with persistent stiffness when throwing. He's being followed for recovery but the visit leads to discovery of a more serious neurological condition.

Erika injures herself while rollerblading with her kids. Initial CT shows bleeding under her abdominal muscles that was to be monitored, but the hematoma expands and she develops active bleeding requiring emergency surgery to ligate the inferior epigastric artery.
