Adult film actor presents with photophobia, muscle spasms, and progressive multi-organ failure. Initially misdiagnosed as meningococcemia and other conditions, ultimately discovered to have disseminated strongyloides (threadworms) throughout his body causing symptoms. However, the worms were actually helping him by keeping his underlying Crohn's disease in check.

The patient's underlying autoimmune condition that was being controlled by the threadworm infection. Developed from being raised in an overly clean environment (hygiene hypothesis). When the worms were eliminated, his Crohn's disease became active and caused rapid multi-organ deterioration including liver failure, cardiac arrest, and bone marrow failure.

Patient developed coagulopathy with blood clotting dysfunction, presenting as petechial hemorrhages and nosebleeds. Initially thought to be from sepsis or meningococcemia, but was actually a complication of his underlying Crohn's disease activated after threadworm treatment.
Also known as: Liver failure
Patient developed progressive liver failure with ascites (fluid accumulation in abdomen), initially suspected to be from sclerosing cholangitis. Actually caused by active Crohn's disease after elimination of protective threadworms. Team discussed potential need for liver transplant.
Also known as: Fluid in the lungs
Patient's lungs filled with fluid after initial treatment with mebendazole for threadworms, requiring oxygen support. This was part of the multi-organ failure caused by uncontrolled Crohn's disease.
