Initially diagnosed as TTP based on ataxia, anemia, and schistocytes on blood smear. Patient underwent splenectomy, but diagnosis was later found to be incorrect when he continued to have symptoms including stroke.

Patient experienced acute stroke with aphasia (word-finding difficulty, nonsensical speech) during recovery from splenectomy, prompting re-evaluation of initial TTP diagnosis.

Final diagnosis revealed patient had developed accessory spleens after traumatic injury 12 years prior. Splenic tissue fragments circulated and implanted throughout body, causing TTP symptoms. Required 16 splenectomies to remove all splenic tissue.

Patient was chronically abusing cough syrup containing DXM to lower his cognitive function and reduce emotional distress from being a genius. Long-term abuse explained his symptoms including kidney failure and paradoxically his cough. He used daily alcohol medicinally to counteract DXM's effects.

Patient developed acute loss of sensation and movement in legs during hospitalization, occurring during withdrawal from DXM after treatment began.
