A real estate developer turned janitor presents with recurrent rash, fever, joint pain, conjunctivitis, hearing loss, kidney failure and coma. The team initially suspects allergic reactions, infections, and tumors before discovering the patient has CAPS (specifically Muckle-Wells syndrome), a rare genetic disorder triggered by cold exposure. The diagnosis is only reached after recognizing the pattern that cold environments (crime scene, MRI room) triggered his symptoms.
