Charlotte goes into labor with triplet girls at 4 centimeters dilation. The labor progresses slowly over many hours. The first baby delivers vaginally without complication, but the second baby is found to be in transverse lie requiring an internal podalic version to rotate and deliver the baby as a breech presentation. All three babies are born premature but healthy.

During Charlotte's triplet delivery, the second baby is positioned sideways in the uterus rather than head-down or breech. Addison performs an internal podalic version to manually rotate the baby and deliver as a breech presentation, avoiding the need for emergency C-section.
