A patient (referred to as 'maiden' in the fairy tale framing) presents with fever, enlarged liver, kidney shutdown, lack of clotting, and internal bleeding. The diagnosis of Wilson's disease is eventually made by recognizing the characteristic Kayser-Fleischer rings (golden rings around the iris caused by copper deposits), and the patient requires a liver transplant.
