Nic sustains a catastrophic traumatic brain injury from a single-car motor vehicle collision. Despite aggressive treatment including ICP monitoring, medications, and medically-induced coma, her brain herniates and she is declared brain dead. This is the central tragedy driving the entire episode.

Gabe spent six months in ICU recovering from severe COVID-19 requiring prolonged intubation, which led to his ongoing tracheal problems. His compromised health from long COVID is central to his current presentation and treatment decisions.

Nic develops significant cerebral edema following her traumatic brain injury, visible on CT scan with petechial hemorrhages in the basal ganglia. The swelling progressively worsens despite aggressive medical management.

After progressive cerebral herniation, Nic is formally declared brain dead following comprehensive brain death examination including assessment of brainstem reflexes and apnea test. This leads to the decision to proceed with organ donation.
